Diagnose perimenopause clinically in women ≥45 with symptoms — FSH not routinely needed
Offer HRT to women with menopause symptoms unless contraindicated
Prefer transdermal oestradiol over oral to minimise VTE risk
Review HRT at 3 months, then annually
Offer testosterone for low libido not responding to HRT (off-label, NICE-endorsed)
Screen for depression and anxiety (PHQ-9, GAD-7) at initial assessment
Assess fracture risk (FRAX) and cardiovascular risk (QRISK3) at initial assessment
Safety-net for unexpected vaginal bleeding, breast changes, severe headaches
